Three years ago in Houston, a new guy on my crew said he could eyeball a 20-ton lift without a signalman. We fired him the same day.

What made him think that was a good idea in the first place? I've seen that kind of overconfidence before, and it usually comes from someone who has never seen a real accident happen. They don't understand that the math doesn't care how good you think your eyes are. A 20-ton load can kill people in an instant if it swings wrong. Firing him immediately was the only safe call.

His training records probably show he passed the safety test but never had a hands-on fail scenario. That gap between paper and practice creates a false sense of security.
