This all sounds very nice and fancy and technical -- but I set everything to auto, use a humongous amount of light, and I 'select' the focal point on the lightest part of the lady's body before I reframe. That's all I do. Most current digital cameras will put everything near and far into fantastic focus no matter what. In fact I occasionally use photoshop to blur backgrounds on my most-favorite gagged-portrait close-ups.
